Requires one 2 by 2 photograph. Requires a $5,000.00 bond good for 1 year for a new statewide license and for 3 years for renewal statewide license dated the day of your hearing.
New statewide license - A first-time application must be renewed after one year and thereafter is renewable every 3 years.
